GEMS pays from your chronic medicine benefit for medicine to treat certain chronic conditions (see Section 3 – Chronic medical conditions) and that are considered clinically appropriate, cost effective and used according to the GEMS protocols.

- Download a chronic medicine application here, or call GEMS on 0860 00 4367 and ask for a form to be emailed to you.
- Your treating doctor must complete the form.
- A separate form must be completed for each member or dependant who needs chronic medicine.
What qualifies as chronic medication?
Acute medication is meant to be used for a short amount of time in order to treat an illness or condition that is short-term, usually less than a month. Whereas chronic medication is meant for several months to years.
